PREPARING FOR YOUR INITIAL VISIT

I need you to complete the Intake and Policy Agreement Form prior to the session because some insurance companies require for pre-authorization to be obtained giving approval for the sessions and how many they approve. I will also need your provider to inform me of any co-pays, deductibles, etc.

The first session typically lasts about 60 sessions, during which we will complete an evaluation of your reasons for seeking a need for change, problems and symptoms you wish to work on, and other personal historical information that will help in determining what type of treatment will best meet your needs.

Prior to your initial appointment, please call Therapist Solutions @ 860-535-9922 find out about your insurance coverage.           

The day of your appointment, please bring all of the Initial Paperwork Packet and the following information:

• Photo ID
• Health Insurance Card
• List of all your current medications, with dosage and prescriber information; as well as any medical or behavioral providers where you are receiving services
• And the following documents are required at time of or prior to initial assessment. I encourage you to print, review, and sign documents ahead of time. If you have a question about the forms, please do not sign it until we meet and I could explain it to you better.
